There is one way to fail on performance
Breaching the maximum drawdown. That is it. There is no daily loss limit, no time limit, and no minimum profit you have to reach by a certain date. If your end of day balance drops below the trailing drawdown line, the account closes.
Consistency is not a way to fail. Exceeding it delays a payout until further trading days dilute it. It does not close an account.
Two things unrelated to performance can also close an account: 7 days without a trade, and prohibited conduct.
If you fail an evaluation
The account stops trading. You have two options:
Reset it. Your account returns to its starting balance and you begin again, keeping the same account. You can do this from your dashboard, and reset prices are listed per program and size on the pricing page.
Buy a new one. Sometimes the better choice if you want a different size or program.
Zenith has no resets. Because it is funded from day one with no evaluation to repeat, a breached Zenith account is replaced by buying again.
If you breach a funded account
The funded account closes and you start again with a new evaluation at the normal price. There is no penalty beyond that.
